After implementing SCIO's program, the ACO achieved greater patient compliance, increased primary care visits and lower inpatient and emergency room utilization among its uncontrolled diabetes and hypertension patient populations. The disease management program resulted in total cost savings of 8 percent for hypertension and 5 percent for diabetes, and the ACO achieved an overall return on investment of 178 percent.
